1. Definition Questionnaire

4. Types of Questionnaire

a)      Open Questionnaire

The open questionnaire is a questionnaire with a description question type. In this case, the respondent needs to provide an answer in the form of an opinion or an explanation in the blank column provided. Open questionnaires are usually used to obtain in-depth information.

b)      Closed Questionnaire

Closed questionnaires are questions that are given in the form of multiple choices or checklists. That way, respondents can only answer questions with the available choices. Closed questionnaire results are relatively easy to process and do not take much time.

c)      Mixed Questionnaire

The mixed questionnaire is a combination of open and closed questionnaire types. Besides being useful for obtaining in-depth information, this method also simultaneously provides a number of data in the form of numbers.

5. How to make a Questionnaire

a)      Define information

This is used as a basic ingredient in designing questions and questionnaire statements.

b)      Formulate questions

Formulate research questions. Make sure the question uses language that is clear, unambiguous, and can be answered properly by the respondent.

c)      Make explanations or follow-up questions

You can develop it from the basic questions that have been made before.

d)      Designing a questionnaire

You need to provide the column for the identity and characteristics of the respondent at the beginning of the questionnaire. After that, arrange the questions in a coherent manner with the type of questionnaire you want. Don't forget to re-evaluate the questionnaire before distributing it.

e)      Doing a pilot test of the questionnaire

Once you're sure, try to do a test run to find out how good the questionnaire is.
